Circle of Strength assist individuals with self-care, social skills, daily living activities, and behavior supports in their homes or community settings according to approved Plans of Care to help them achieve their goals and dreams .
Circle of Strength Community Inclusion services; empowers individuals with disabilities through recreational, educational, and community-based programs to enhance skills, self-esteem, social skills, and confidence. The services aim to support their long and short-term goals while encouraging safe participation in the community.
Circle of Strength provides community-based services for individuals with developmental disabilities to support their full community participation and realization of personal goals. The Support Program caters to adults aged 21 and above living with families or in other settings.
Circle of Strength provides short-term respite services to individuals who cannot care for themselves, offering relief to their regular caregivers. Our goal is to help individuals transition to group homes or independent living, ensuring their safety and security with support from our staff.
Circle of Strength provides transportation to our participants to assist them with their doctor's appointment, work, school etc. Assisting individuals and families the opportunity to work with an individual that they are comfortable and consistent with.
Circle of Strength supports with Behavioral Supports, assessments, behavioral modification plans, training, counseling, and interventions for individuals with developmental disabilities to improve, acquire or maintain appropriate interactions with others.
Serves are available to individuals who are ages 18 or older and meet all other eligibility requirements.
-U.S. Citizen and resident of New Jersey
-Eligible for or enrolled in medicaid
-Complete and submit DDD application
-Intake processes with DDD
-A trained division facilitator will schedule a face-to-face meeting with you.
-The facilitator will submit the completed NJCAT to the Rutgers University Developmental Disabilities Planning Institute (DDPI), where the scores are tabulated and the tier is established.
-The division will mail a tier letter to eligible individuals within four weeks of the face-to-face meeting.
-An organization that will help you get the services you need.
-Fill out support coordination agency form.
-Choosing a support coordination agency or one will be appointed to you.
-Interviewing support coordination agency.
-Support Coordination will go over your budget and ISP (Individual Specialized Plan)
Once your plan is approved, services begin with your Direct Support Agency:
-Circle of Strength.
