Support Coordination

Njcat- Assessment

 

Division services are available to individuals who are age 21 or older and meet all other eligibility requirements.

Individuals receiving services through school are able to continue doing so through age 21. If you choose to graduate or leave school between the ages of 18 and 21, it is critical to discuss service options with your school’s Individualized Education Program (IEP) Team and transition coordinator.

Njcat- Assessment

 A trained Division facilitator will schedule a face-to-face meeting (virtually during the COVID-19 pandemic) with you and your guardian, if you have one. It can be helpful to have additional people participate in the meeting who know you and spend time with you – for example, other family members, service providers and caregivers 


The most important in the process of receiving your DDD benefits are answering the questions on the NJCAT, please inquire with our team to help understand the questions and the process

 Support Coordination is a Division-funded service that assists individuals in gaining access to needed program and State plan services, as well as needed medical, social, educational and other services. 


Choosing a Support Coordinator is important. We can assist with helping to find the right fit for you and your family. Discussing the right questions to ask, and working together to talk about the process.

Circle of Strength can assist with the Direct Support Services along with onboarding your family or friend as a paid live in caregiver or get assistance with a DSP-Direst Support Professional to help with your loved ones.
